I would advise that breaking down the parts of an EHR into smaller components might be one way to gradually ‘eat the whole elephant’ of building an EHR.

Is the SMART on FHIR a viable approach?

I have to admit I’ve not seen it in practice, I don’t know the limitations, and I don’t know even if the standards are truly open/compatible with an open source effort?
