Does anyone have a copy of the original list of these patients? 

It probably came from HSCIC or NHS Digital .... .and I can't find it. I can many systems are using it for testing

Yes. Work in progress https://nw-gmsa.github.io/testing.html#test-patients

Looks like we will be adding our own for the north west (which logically should be combined with Londons)
